What is Gastric Bypass ?

Gastric Bypass is a bariatric surgical method based on reducing the volume of the stomach and changing the intestinal functions. With this method, food intake and nutrient absorption are reduced and the patient loses weight. In Gastric Bypass surgery, a small pouch is created in the stomach and this pouch is directly connected to the small intestine. After the surgery, the foods eaten by the patient first move to the stomach, which remains in small volume, and then to the small intestine.

Gastric Bypass  surgery  can be performed with both open and closed techniques, but the closed technique has been preferred in recent years. Gastric Bypass operation performed with the closed technique consists of two stages. In the first stage, a small part of the stomach is turned into a chamber by entering the abdominal cavity with the help of surgical instruments through small holes opened in the abdomen.

Then this small chamber and most of the stomach (about 90%) are separated from each other, but most of the stomach is not removed from the body, it is simply deactivated. In the second stage, the small intestine is shortened by 50-75 cm and its connection with the formed small stomach is provided. The operation takes approximately 1-2 hours. With this surgery,  nutrient intake and nutrient absorption of obesity  patients are reduced.

Gastric Bypass surgeries are of 3 types according to the type of operation. These types of surgery are:

  1. Roux-En-Y (Proximal):  It is the most commonly used gastric bypass operation method. With this method, most of the absorptive functions of the small intestine are preserved and after the surgery, the person feels full in a very short time when he eats. It has very few nutritional side effects.
  2. Roux-En-Y (Distal):  In this method, the structure of the small intestine is changed. This change in structure causes insufficient absorption of fats and starches, and sometimes vitamins. It is a method in which side effects are seen more in terms of nutrition.
  3. Loop Gastric Bypass (Mini Gastric Bypass):  With this method, stomach capacity is reduced and small intestine absorption is reduced. The duration of the surgeries performed with this method is short.

Advantages of Gastric Bypass A surgery :

  • Since the stomach is reduced by surgery, both food intake is restricted and nutrient absorption is reduced. Thus, especially fat and sugar absorption is prevented and the weight loss process of the patient is accelerated.
  • Since the stomach is reduced after the surgery, the patient eats less and feels full faster.
  • The risks of diseases such as diabetes, heart disease risk, sleep apnea, high blood pressure due to excess weight are reduced or eliminated.
  • Since the area where the stomach, which is reduced during the operation, connects to the intestine is not large, the patient feels full for a long time.

Disadvantages of Gastric Bypass Surgery :

  • Vitamin and mineral deficiencies may occur in patients who do not comply with the nutritional program after surgery. Conditions such as anemia, hair loss, and decrease in muscle mass may occur due to these deficiencies.
  • After gastric bypass operation, patients may experience nausea, constipation and intestinal problems.
  • Hernias may occur in the surgical entrance areas.
  • Gastric Bypass does not guarantee weight loss or weight regain.

Who Can Have Gastric Bypass Surgery ?

Gastric Bypass surgery  can be performed on people between the ages of 18-65 who meet certain conditions. The first condition for gastric bypass operation is that the patient’s body mass index should be 40 and above, or the body mass index should be between 35-40 and there should be co-morbidities related to obesity. The second condition of the operation is that the patient has diseases such as diabetes and reflux and the need to lose weight faster. Parental consent is required for patients under the age of 18 to perform surgery.

Nutrition After Gastric Bypass Surgery

Things to consider in nutrition after gastric bypass surgery can be listed as follows:

  • At least 3 meals a day should be fed and good foods should be consumed.
  • In meals, protein, fruit and vegetables, whole-wheat cereals should be given importance.
  • Liquid nutrition , especially in the first 2 weeks after surgery  , 3-5. Puree nutrition in weeks is very important.
  • At least 1.5-2 liters of water should be consumed per day.
  • Simple sugars and sweets should be avoided.
  • It is necessary to chew the food very well, especially after it is pureed and swallowed. Foods that are not well chewed can cause pain and vomiting.
  • Make sure you get enough protein every day.
  • Solid and liquid foods should not be consumed together. Liquids consumed with meals can cause rapid satiety or prevent the feeling of fullness, causing you to eat more.
  • Liquids should not be consumed at least 30 minutes before and after meals.
  • Food should be consumed very slowly. A meal should be consumed between 20 and 45 minutes on average.
  • When fullness is felt, excessive consumption of food should be avoided.
  • It is important to note the consumed foods and share them with the doctor.
  • After the surgery, especially fresh bread, rice pilaf, pure meat, fibrous (celery, sweet corn) and raw fruits, acidic fruits (orange, grapefruit, etc.) should not be consumed. Instead of these foods, toast, crackers, rice soup, small and slow-cooked meats, skinless and well-cooked broccoli, cauliflower and tomatoes, peeled fruit or diluted fruit juices can be consumed.

The foods that can be consumed according to the weeks after the surgery are:

  • Week 1 :  Meat broth, chicken broth, apple juice (diluted), vegetable juice (diluted), unsweetened compote, buttermilk made from skim milk and fat-free yogurt, protein powder and unsweetened herbal tea can be consumed.
  • Week 2 :  In addition to the foods in the first week, soups without grains, which have been passed through a blender, can be consumed. Non-spicy soups, fish soup, chicken broth noodle soup (without grains), tomato soup (without tomato paste), yogurt soup and lentil soup can be consumed.
  • 3. Week:  In addition to the foods of the first two weeks, soft and crushed foods can be consumed. In puree food consumption, care should be taken to include protein-calcium-based foods. Eggs, vegetable purees, nonfat yogurt and curd cheese, fruit purees (can be consumed with milk or yogurt), tuna fish, chicken, mashed potatoes can be consumed.
  • 4th and 5th Weeks:  In addition to the other weeks, foods that can be crushed with a fork are recommended. Vegetable puree with mushrooms, wedding soup, seasoned meatballs and cheese omelet can be consumed.
  • Week  6 : Dietary products rich in protein and low in calories should be consumed. It is possible to switch to solid food consumption. Protein should be consumed every day. Cooking meals in chicken and broth helps to meet protein needs.

Gastric Bypass Surgery Risks

As with any surgical operation, Gastric Bypass surgery also carries some risks. These risks are listed as follows:

  • Bleeding, infection and postoperative intestinal obstruction, hernia and general anesthesia complications may occur during the surgery.
  • The biggest risk of Gastric Bypass operation is leakage in the stomach and small intestine connection. This leak may require a second surgery. Secondly, Gastric Bypass surgeries can cause blood clot formation and cardiac problems in the legs and lungs.
  • About 10-15% of patients who have surgery experience some of these complications. Serious complications during or after this surgery are very rare. Complications that usually occur and are common are those that are simpler and treatable.

Differences Between Gastric Bypass and Gastric Sleeve

Although gastric bypass and sleeve  gastrectomy  surgeries are similar in terms of being applied to obesity patients, they are basically different surgeries. The differences between these two surgeries are as follows:

  • In gastric sleeve surgery, the intestines are not interfered with, while in gastric bypass surgery, the stomach is connected to the intestines for a short time.
  • In sleeve gastrectomy surgery, the stomach is shrunk and left in the shape of a banana, but in Gastric Bypass surgery, almost 90% of the stomach is disabled and the food passes through the shrunken stomach and goes directly to the small intestine.
  • While the digestive system works regularly after sleeve gastrectomy, some digestion steps are skipped after Gastric Bypass operation.
